A person receiving palliative care may also have an assessed need for home nursing or therapy. HarvardCare's role must be coordinated with the treating palliative team and the patient's goals; this page does not establish a standalone palliative or hospice program.

Patient-goal discussion

Home health may support a person who is also receiving palliative care, but the responsibilities must be clear. HarvardCare's role is the specific skilled service accepted under an individual plan. This page does not establish a dedicated palliative-care or hospice program, and overlapping clinical and coverage arrangements require review with the involved providers.

Appropriate functional support

The patient's goals should shape appropriate functional care. An ordered therapy assessment may focus on a manageable daily task or caregiver assistance within the current condition. Nursing follows relevant clinical orders, while medication and symptom-treatment decisions remain with the responsible treating clinician. The aim is a coordinated plan that reflects the patient's preferences, without promising a particular symptom result.

Palliative-team coordination

Caregivers may need practical or emotional support, which relevant clinicians or ordered social work can assess within their scopes. Outside resources remain subject to confirmation. Intermittent visits do not provide continuous bedside attendance or emergency-response coverage. The referral should identify the treating palliative team and the exact skilled need being requested.

Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
