Fall Risk Assessment at Home: an individual home care plan

Fall risk can emerge during a routine that seems familiar: dressing near a bed, entering a shower or reaching into a cupboard. An OT assessment examines how the person, task and surroundings interact in those activities.

Daily-task risk review

OT fall risk assessment concentrates on the interaction between a person and an activity. Dressing beside a bed, reaching above a counter and moving into a shower create different demands. The therapist asks about falls and near-falls, then considers the relevant task, equipment, environment and assistance within the patient's clinical context.

Environmental assessment

The assessment may identify a need for task instruction, a setup change or further coordination. A bathroom concern can involve both the transfer and the way clothing is managed. Balance or gait impairments may warrant PT input, while dizziness or medication concerns belong with the appropriate clinician. These are related contributors, not interchangeable explanations.

Equipment-use observation

Recommendations need to work for the actual household. Shared spaces, property limits and the caregiver's abilities can affect what is practical. A completed assessment does not certify a fall-proof home. The patient and family should understand the remaining assistance needs and when changes in the routine or condition call for another review.

Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
