In-Home Physical Therapy in South Whittier
South Whittier is an unincorporated stretch of postwar tract housing between Whittier and Norwalk ā the Leffingwell and Colima Road corridors, Santa Gertrudes and Mills, the Sorensen Avenue and Norwalk Boulevard blocks, and the Lambert Road and Carmenita Road areas. It is flat, settled, and ageing in place, and a significant share of the patients we see here are living at home with memory loss. Physical therapy for those patients is a genuinely different discipline, and our licensed physical therapists practise it differently.
Our therapists serve all South Whittier residential neighborhoods, the Leffingwell Road and Colima Road corridors, Santa Gertrudes Avenue and Mills Avenue, the Sorensen Avenue and Norwalk Boulevard blocks, and the Lambert Road and Carmenita Road areas.
Therapy When the Patient Cannot Remember the Instructions
A standard home program relies on the patient recalling a sequence between visits. With dementia that assumption fails, and adding more instructions makes it worse rather than better. The work has to change shape.
In practice that means single-step instructions given one at a time and demonstrated rather than described, the same words and the same order at every visit because familiarity does the work memory cannot, and sessions at the hour of day when the patient is most settled ā usually morning. It means changing the environment rather than expecting the patient to adapt to it: a clear path, the walker in the same place every time, the bathroom door left open, a chair where the rest is needed. And it means the caregiver is the person we are really training, because they will be delivering the program six days out of seven.
We are also honest about what therapy can and cannot address. Walking, transfers, and strength can be worked on and often improve. Judgment about when to ask for help usually does not, which is why the environment has to carry the safety rather than the patient’s decision-making. We say that plainly to families rather than leaving them to discover it.
What the Evaluation Covers
Your therapist measures strength, joint range of motion, standing balance, and gait speed, observes how instructions are followed, and measures the home: the entry steps, the clear path between the rooms in daily use, doorway widths against the device, furniture and bed heights, the bathroom, and exits and how they are secured. Medications, blood pressure response to standing, and fall history are reviewed.
Conditions We Treat at Home
- Mobility and transfer training with cognitive impairment
- Fall prevention and environmental simplification
- Recovery after knee, hip, and shoulder replacement or repair
- Gait retraining after stroke or with Parkinson’s disease
- Weakness and deconditioning after a hospital stay
- Back and joint pain limiting daily activity
- Caregiver training and support for the household
The Caregiver Is Also the Patient’s Best Asset ā and at Risk
A spouse or adult child providing daily care for someone with memory loss is doing a job that runs continuously and has no obvious end, and they frequently arrive at our first visit exhausted and not sleeping. We train them properly on transfers so they are not injuring themselves, and we say clearly when the honest answer is that more help is needed rather than better technique. Where that is the case we ask your physician for a medical social work referral, which our agency provides, because a caregiver who breaks down is the most common reason a patient leaves home altogether.
Coordination With Your Physicians
We coordinate with Whittier Hospital Medical Center, PIH Health Whittier Hospital, Los Angeles Community Hospital at Norwalk, Kaiser Permanente Downey Medical Center, PIH Health Hospital – Downey, and La Palma Intercommunity Hospital. Your physician receives objective measurements and progress notes after visits.

Starting Physical Therapy Services
Home physical therapy requires a physician order documenting medical necessity. Medicare may cover in-home physical therapy if you qualify, which generally requires a physician order, a skilled therapy need, and homebound status as Medicare defines it. Coverage is not guaranteed in advance. We accept Original Medicare Part A & B, Blue Shield of California PPO, and Anthem Blue Cross PPO.
Our intake team verifies your coverage and explains your benefits before services begin. Most patients start within one to two days of a completed referral.

Tell the therapist which instructions are confusing and what the person responds to. The plan can be adapted to the assessment and caregiver support available. Consistent cues may help with participation, but they do not replace the supervision or assistance the person needs.
