Post-surgical incision care and rehabilitation share the same starting point: the surgeon's current instructions. Nursing can address ordered incision needs while PT and OT plan movement and personal activities within those directions.

Surgeon-order review

Incision-related nursing starts with the surgeon's current directions. The procedure details, wound instructions and relevant activity or bathing restrictions give the home team a common reference. Conflicting verbal and written instructions need clarification from the surgical team before an uncertain method is followed.

Incision assessment and teaching

Nursing and rehabilitation have related but distinct responsibilities. The nurse addresses accepted ordered incision care and teaching, while PT and OT assess appropriate movement or personal-care tasks. The therapist does not revise the incision treatment during an exercise visit. Decisions about sutures, staples or drains require the responsible clinician's direction and appropriate qualified care.

Bathing precaution coordination

Family members can participate in tasks that have been explicitly assigned and taught. They should also understand the surgical contact and emergency instructions for new concerns. Not every operation requires home nursing, and an accepted referral does not promise a healing date. The plan follows assessed needs and review, with surgical follow-up remaining part of care.

Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
