MOVEMENT & DAILY LIFE

Physical Therapy for Your Everyday Goals

Your therapist starts with what matters in your home: reaching the bathroom, rising from a chair, or feeling steadier on your feet. These are examples of goals a personalized therapy plan may address; a diagnosis alone does not establish Medicare eligibility.

Walking & Mobility

Practice moving through familiar rooms, turning, and using a prescribed walking aid. Your therapist considers your pace, surroundings, and the help you need.

Recovery After Surgery

Work toward daily movement goals after surgery with activities adapted to your provider’s precautions, current strength, and stage of recovery.

Balance & Fall Concerns

Review situations that make you feel unsteady and practice appropriate balance activities. The therapist can also discuss movement habits and obstacles in your home.

Strength After Illness

Build a plan around weakness or reduced activity after an illness or hospital stay, with exercises and rest periods matched to how you are doing.

Support for Lasting Changes

Therapy may address movement changes after a stroke or with a chronic condition. Goals can include maintaining function when a clinician’s skills remain necessary.

Transfers & Home Routines

Practice moving between a bed, chair, and standing position. With your permission, a caregiver can learn ways to support your prescribed plan.

YOUR THERAPY VISIT

What Your Physical Therapist Brings to Each Visit

Your therapist connects the clinical plan to the rooms, routines, and challenges you live with every day. Visits may include these elements, based on your assessment and orders:

1

An Assessment of Your Movement

Your therapist asks about your goals and assesses relevant areas such as walking, strength, balance, and getting up from a chair.

2

A Plan for Practical Goals

Together, you identify meaningful activities to work on. The therapist selects tasks that fit your ability and follows your provider’s precautions.

3

Guided Practice at Home

Practice prescribed exercises and everyday movements with feedback on technique, pacing, and using your available space or equipment.

4

Instructions You Can Follow

Ask questions about your home program and what to do between visits. Your therapist explains how a family member or caregiver can help when appropriate.

5

Review & Provider Updates

The care team reviews your needs over time, shares relevant changes with your provider, and adjusts the plan or next steps when appropriate.

HOW THE BENEFIT WORKS

Understanding Medicare Coverage for Home Health PT

Home health PT and outpatient PT have different Medicare rules. This page describes therapy through a Medicare-certified home health agency. We review your situation before confirming whether our services fit your coverage.

$0
For Covered Home Health Visits

Eligible Original Medicare patients pay nothing for covered home health visits. Covered equipment usually has a 20% cost share after the Part B deductible.

A qualified provider assesses and orders care
You meet Medicare’s homebound requirements
You need medically necessary skilled therapy
A certified agency delivers your ordered plan

Need help understanding these requirements? Submit the form so we can review your needs, provider information, and available services.

Know Which Benefit Applies

Homebound generally means you normally cannot leave without considerable effort and need help, or leaving is medically inadvisable. Medical visits and short, infrequent outings can still be allowed.

Original Medicare Home Health Your provider must complete the required face-to-face assessment and certify your need for home health. Our team reviews the orders and supporting information.

Other coverage questions:

Blue Shield of California PPO / Anthem Blue Cross PPO Tell our intake team about both plans so your coverage can be reviewed together.
What About Outpatient PT? Outpatient PT under Part B generally has 20% coinsurance after the deductible. A visit taking place at home does not, by itself, make it a home health benefit.

Time for Your Questions

Tell us what feels difficult, what matters to you, and which instructions need another explanation. Your comfort and understanding matter.
