We Pull on the Handrail Before We Rely on It

Private hillside stairs here are often decades old, built of wood or of concrete with a wooden rail, exposed to sun and rain the whole time, and maintained when somebody gets around to it. Posts loosen in their footings. Treads soften. A rail can look entirely sound and move an inch when you put weight on it.

That is worse than no rail at all, because a patient commits their balance to it. So part of the first visit is a physical check: we load the rail, look at the posts, check the treads, and note where the rail stops short of the bottom step, which is extremely common and is exactly where the last and worst fall happens. If the rail is not sound, we say so immediately, teach a technique that does not depend on it, and put the repair in writing to your physician. If it is sound, that is worth knowing too, because then we can build the program around using it properly.

What the Evaluation Covers

Your therapist measures strength, joint range of motion, standing balance, and gait speed, then measures the property: the stair count, rise, and rail condition, where the rail begins and ends, the surface underfoot, the distance from the door to where a car can stop, doorway widths against your device, furniture and bed heights, and the bathroom. Medications, blood pressure response to standing, and fall history are reviewed.

The Errands That Use the Stairs

It is rarely the stair by itself that causes trouble. It is the stair while carrying something — a bin out to the street, groceries in, a laundry basket — with both hands full and no hand free for the rail. We train carrying as its own progression rather than assuming it returns with general strength, and in the meantime we are practical: a bag that goes over a shoulder, two trips instead of one, a neighbour who takes the bins, a rolling cart at the top. Where the stairs cannot be managed safely at all right now, we say so plainly and document it, because that is what supports getting help arranged rather than leaving someone to improvise.
