In-Home Physical Therapy in Green Meadows
Green Meadows covers the blocks around the recreation center and 89th Street, the Central and Compton Avenue corridors, Avalon Boulevard and the streets west of it, and the homes off 92nd Street and Century Boulevard. It is flat and densely built, with small single-story houses. A significant number of the patients we treat here are on dialysis, and that imposes constraints on a therapy program that are easy to get wrong. Our licensed physical therapists plan around the treatment schedule rather than against it.

Therapy Around a Dialysis Week
Three sessions a week, three or four hours each, plus transport, consumes most of three days. What is left is not evenly distributed: the hours after treatment often bring real fatigue and a drop in blood pressure, which makes that the worst possible time for a therapy session ā and frequently the time a scheduler picks, because the patient is known to be home.
We schedule for the non-dialysis days and the mornings before treatment, and we set exertion with the fluid cycle in mind, since a patient two days from their last session is carrying more fluid and works harder for the same result. Blood pressure is checked with position change rather than assumed.
There is also a hard physical constraint that gets missed. An arm with a fistula or graft must not take weight and must not be used for a blood pressure cuff ā which rules out pushing up from a chair with that arm, bearing weight through it on a walker, and several standard transfer techniques. We plan around it explicitly and make sure the household knows, because a well-meaning relative pulling on that arm during a transfer is a serious problem.
What the Evaluation Covers
Your therapist measures strength, joint range of motion, sitting and standing balance, gait speed, and exertion tolerance, notes access-site precautions, and measures the home: chair and bed heights for a rise without using the affected arm, doorway widths against your device, the bathroom, the entry steps, and the route out for transport. Medications, blood pressure response to standing, and fall history are reviewed.
Conditions We Treat at Home
- Conditioning and transfers for patients on dialysis
- Weakness and deconditioning after a hospital stay
- Balance deficits and fall prevention
- Recovery after knee, hip, and shoulder replacement or repair
- Gait retraining after stroke or with Parkinson’s disease
- Back and joint pain limiting daily activity
- Caregiver training for the household, in Spanish or English
Getting to Treatment Is Part of the Goal
Missing dialysis is dangerous, and mobility is one of the reasons people miss it ā the walk to the transport, the transfer into the vehicle, the wait, and the walk back in afterwards when they are at their most depleted. We treat that whole route as a functional goal, measure the distance and the transfer, and report to your physician when the limitation is what is putting attendance at risk, because that is often what supports arranging transport that can accommodate you.
Coordination With Your Physicians
We coordinate with MLK Community Hospital, St. Francis Medical Center, Memorial Hospital of Gardena, Community Hospital of Huntington Park, Kindred Hospital South Bay, and Kedren Community Health Center. Your physician receives objective measurements and progress notes after visits.

Starting Physical Therapy Services
Home physical therapy requires a physician order documenting medical necessity. Medicare may cover in-home physical therapy if you qualify, which generally requires a physician order, a skilled therapy need, and homebound status as Medicare defines it. What you pay depends on your specific plan, and coverage is not guaranteed in advance. We accept Original Medicare Part A & B, Blue Shield of California PPO, and Anthem Blue Cross PPO.
Our intake team verifies your coverage and explains your benefits in Spanish or English before services begin, and we can contact your physician’s office directly to request the order.

Coordinate the timing with the therapist and dialysis team. Fatigue, blood-pressure changes, transport, and how you feel before and after treatment can influence the plan. There is no single best schedule for every dialysis patient, and a change in symptoms may require the activity plan to be reassessed.
Share the specific access-care and activity instructions from your dialysis team. The therapist can adapt transfers, exercise, and equipment use to those precautions and clarify uncertainties with the treating team. Do not assume a general rule about arm use applies to every access type or stage of healing.
That is often the main goal. The walk to transport, the transfer, and the walk back in afterwards are what cause missed sessions, so we measure that route and report to your physician when mobility is what is putting attendance at risk.
Tell the therapist and dialysis team about the change, including fatigue, dizziness, or difficulty standing. Do not push through symptoms to complete a planned session. Your clinicians can help determine whether the exercise amount or timing needs adjustment and whether a medical review is needed.
