Any one of those can exceed what someone can do two weeks after a hospital stay, and none of them are measured before discharge.

We measure them. The evaluation covers the route out of your building and the realistic path to a car, and the program treats that route as a goal with stages — because being able to reach your own physician is the difference between a recovery that continues and one that stalls. Where the route cannot be managed yet, we say so and document it, which is often what makes a home visit from another provider possible in the interim.

What the Evaluation Covers

Your therapist measures strength, joint range of motion, standing balance, and gait speed, then measures the setting: the stairs or elevator from your unit, the courtyard or walkway to the street, doorway widths against your device, furniture and bed heights, the bathroom transfer, and the distance to a workable pickup point. Medications, blood pressure response to standing, and fall history are reviewed.

A Home Program You Cannot Read Is Not a Program

This is one of the most linguistically varied neighborhoods in the county, and a printed exercise sheet in English handed to a household that speaks Armenian, Spanish, or Thai accomplishes nothing. Our staff includes Spanish-speaking clinicians, and where we cannot deliver instruction directly in a patient’s language we arrange interpretation rather than proceeding and hoping. We also check comprehension by having the patient or caregiver perform the exercise back rather than by asking whether they understood — which almost always produces a yes regardless.

Coordination With Your Physicians

We coordinate with CHA Hollywood Presbyterian Medical Center, Kaiser Permanente Los Angeles Medical Center, Southern California Hospital at Hollywood, Barlow Respiratory Hospital, and PIH Health Good Samaritan Hospital. For patients coming home after a hospital stay we work directly with discharge planners. Your physician receives objective measurements and progress notes after visits.

Medicare may cover in-home physical therapy if you qualify, which generally requires a physician order, a skilled therapy need, and homebound status as Medicare defines it.
