Self-care task assessment

An ADL assessment follows a personal-care activity from preparation through completion. A person may manage the final movement but struggle to organize supplies or begin the sequence. Another may understand every step yet need physical help at one point. The therapist identifies these differences through discussion and appropriate observation rather than assigning a standard self-care checklist.

Activity sequencing

Training may address position, task order, equipment or the assistance used. Dressing with a movement restriction is a different problem from bathing with limited activity tolerance. The method must fit the individual task and the patient's preferences. Caregivers can learn where cues are useful and where hands-on help is still necessary.

Adaptive methods

The practical plan should distinguish skilled training from the assistance needed between visits. An OT visit does not supply continuous personal care or establish that a family member can manage every task. Changes in medical restrictions, cognition or helper availability should be discussed so the routine can be reviewed against current needs.
