Fall-history review
Fall-prevention assessment works best with a description of what actually happened. A trip while approaching a rug differs from losing balance during a turn or feeling unsteady after standing. Near-falls matter too. The therapist uses those details with movement assessment to understand which tasks and circumstances need attention.
Balance during daily tasks
PT may examine balance during a transfer, a change in direction or an appropriate reach. The home layout, usual footwear and mobility device are relevant because these are the conditions in which the person moves between visits. OT or nursing may contribute when environmental routines, personal care or clinical concerns also affect the situation.
Turning and reach assessment
A written plan needs to distinguish therapist-guarded practice from activities appropriate between visits. Families should understand the agreed assistance and avoid adding challenges simply because an activity looked manageable during a session. Fall risk has several possible contributors; no exercise, device or household change can guarantee that a fall will not occur. Review focuses on the individual tasks and support needs identified in assessment.

Eligibility and insurance review
Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.

PT examines movement, balance and the assistance needed during activities. Environmental changes may be part of a broader plan, but rearranging a room alone does not assess a person's mobility.
Turning and changing direction can be assessed within the person's abilities and precautions. The therapist may compare tasks in open space with the demands of the actual household route.
What is worn or used during movement can affect how a task is performed. Bring the usual footwear and device to the assessment instead of demonstrating with unfamiliar equipment.
Practice only the activities and level of supervision specified in the plan. A task used during a therapist's guarded assessment is not automatically suitable for independent practice.
The therapist can explain the agreed assistance, setup and cues for that task. Caregivers should also know when the activity is beyond the help they can safely provide.
The team can review the route, transfers and circumstances around urgency. Nursing or OT may also be involved when the issue extends beyond movement assessment and PT instruction.
Report dizziness, drowsiness or other concerns to the treating team. Medication review belongs with the appropriate clinician; a therapist does not independently change prescribed medication.
Review may consider the assistance, stability and consistency observed during selected tasks. A period without a fall does not by itself establish that every household activity is safe.
Describe the episodes and current mobility to the ordering clinician or intake team. The next step is assessment of skilled needs and home health eligibility, rather than a guaranteed prevention program.
