Home Safety Evaluation: an individual home care plan

A home safety evaluation looks beyond a checklist of loose rugs. The therapist considers entrances, room arrangements and daily routines in relation to the patient's current abilities, equipment and assistance.

Room and route review

A functional home safety review begins with the rooms and routes the patient actually uses. Entrance access, the sleeping area, bathroom and kitchen may present different problems depending on mobility, vision, cognition and equipment. The therapist considers how activities are performed in these spaces rather than evaluating the building as if it were empty.

Task-specific safety assessment

Recommendations can concern a task method, furniture placement or an appropriate support. More substantial changes may require property permission and qualified installation. OT does not certify structural, electrical or code compliance. Separating those responsibilities helps the household understand what a clinical recommendation establishes and what still requires another professional.

Access recommendations

The usual arrangement should be explained before changes are made solely for the visit. Shared household needs, pets and caregiver access may all affect an otherwise simple suggestion. After equipment or furniture changes, functional use may need review. A safety plan should remain connected to the patient's current abilities rather than being treated as a permanent checklist completed once.

Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
