In-Home Physical Therapy in West Adams

West Adams holds some of the oldest and grandest housing in the city — Lafayette Square and Victoria Park, West Adams Heights and Sugar Hill, Kinney Heights and Harvard Heights, Wellington Square, the Washington Boulevard corridor, and the blocks between Adams and the freeway. It is also a neighborhood in the middle of a long restoration, and a great many of our patients here are recovering in a house that is partly a building site. Our licensed physical therapists treat that as a real clinical variable, because it is.

Our therapists serve Lafayette Square and Victoria Park, West Adams Heights and Sugar Hill, Kinney Heights and Harvard Heights, Wellington Square and the Washington Boulevard corridor, and the blocks between Adams Boulevard and the Santa Monica Freeway.

Recovering in a House Under Renovation

Restoration means an exposed subfloor with a lip at every doorway, a staircase with the balusters removed and no rail at all, extension cords across hallways, dust that matters a great deal to anyone with a lung condition, tools left where they were last used, and a bathroom that is out of service for three weeks. Discharge planning assumes none of this.

We work with it rather than around it. The first visit establishes what the house will actually look like over the next month and what is due to change, because a program built around a bathroom that is about to be demolished is wasted. We agree which single route through the house stays clear and rail-equipped while the work goes on, and we are direct with the household about the few things that genuinely cannot wait — a temporary rail on an open staircase is not optional, and neither is a cleared path from the bed to whichever bathroom is functioning. Where dust is a problem for a patient with COPD or asthma, we say so plainly to the household and to your physician.

What the Evaluation Covers

Your therapist measures strength, joint range of motion, standing balance, and gait speed, then measures the house as it currently is: which staircases have rails, floor levels and exposed thresholds, which bathroom is in service, cords and obstructions, doorway widths against your device, furniture and bed heights, and the entry steps. Medications, breathing, blood pressure response to standing, and fall history are reviewed.

Conditions We Treat at Home

  • Recovery after knee, hip, and shoulder replacement or repair
  • Staircase retraining in large older homes
  • Balance deficits and fall prevention
  • Gait retraining after stroke or with Parkinson’s disease
  • Weakness and deconditioning after a hospital stay
  • Back and joint pain limiting daily activity
  • Caregiver training for the household

Big Houses, Long Distances

Even finished, these are large homes: the walk from a back bedroom to the front door can be sixty or seventy feet before anyone goes outside, and the main bedroom and bathroom are usually upstairs. That distance is a resource once the route is safe — the interior itself becomes the graded distance program, measured down the hall and tracked in feet rather than in a general sense of improvement. We also work through the question of whether to set up downstairs temporarily, which is not automatically right: it often trades a staircase for a bathroom that does not work and a sofa at the wrong height.

Coordination With Your Physicians

We coordinate with Kaiser Permanente West Los Angeles Medical Center, Crenshaw Community Health Center, Southern California Hospital at Culver City, Kindred Hospital Los Angeles, Cedars-Sinai Medical Center, and California Rehabilitation Institute. Your physician receives objective measurements and progress notes after visits.

Starting Physical Therapy Services

Home physical therapy requires a physician order documenting medical necessity. Medicare may cover in-home physical therapy if you qualify, which generally requires a physician order, a skilled therapy need, and homebound status as Medicare defines it. Coverage is not guaranteed in advance. We accept Original Medicare Part A & B, Blue Shield of California PPO, and Anthem Blue Cross PPO.

Our intake team verifies your coverage and explains your benefits before services begin, and same-week visits are usually available once a referral is complete.

Lafayette Square and Victoria Park, West Adams Heights and Sugar Hill, Kinney Heights and Harvard Heights, Wellington Square and the Washington Boulevard corridor, and the blocks between Adams and the Santa Monica Freeway.

Yes, and we plan around what the house will actually look like over the next month. A program built around a bathroom that is about to be demolished is wasted, so we establish what is changing and when.

A temporary rail on an open staircase, and a cleared path from the bed to whichever bathroom is in service. We are direct about those, and about dust where a patient has a lung condition.

Not automatically. It often trades a staircase for a bathroom that does not work and a sofa at the wrong height. We walk both options and give a recommendation with the reasoning attached.

Describe the current routes, temporary barriers, dust, and which bathroom or sleeping area remains accessible. The plan may need adjustment as work progresses. Notify the team about changes instead of continuing to practice on a route that has become obstructed.
