In-Home Occupational Therapy: an individual home care plan

Occupational therapy looks at how a person carries out daily life. Getting washed, choosing clothing, organizing a task and using household equipment may each need a different approach after illness, injury or a change in function.

Daily-activity assessment

OT begins with the activities that make up the patient's day. The therapist considers what the person wants to do, what currently requires help and where a routine becomes difficult. Movement, hand use, attention and the home environment may all contribute to the same task. An assessment should examine that combination rather than assume the diagnosis explains everything.

Task adaptation

Daily-activity practice can involve a different method, appropriate equipment or clearer organization of the task. The actual clothing, bathroom and supplies matter because they affect how the person participates. Recommendations should respect privacy and preferences as well as clinical restrictions. An aid is not automatically suitable simply because it is marketed for older adults or rehabilitation.

Equipment instruction

Caregiver instruction connects the skilled visit with ordinary routines between visits. The helper needs to understand which assistance is appropriate and where another arrangement may be needed. When PT, nursing or speech therapy is also ordered, OT can share relevant observations and restrictions. That coordination keeps activity advice connected to the full recovery plan.

Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
