A discharge packet can contain several sets of instructions. Home rehabilitation brings movement practice back to the surgeon's orders and the tasks waiting at home: reaching a bathroom, changing position and using the entrance safely with the prescribed assistance.

Surgical precaution review

Post-surgical rehabilitation begins with the operation and the current orders. Weight-bearing directions, movement precautions and device recommendations must be understood before a therapist selects an activity. If the documents disagree, the discrepancy needs clarification from the surgical team. A familiar exercise from an earlier operation should not be carried into the new plan without review.

Transfers after discharge

The assessment then turns those instructions into household tasks. Getting into bed, reaching the toilet or using an entry step each combines several movements. The therapist considers the surface height, available support and the person helping. A household may need to discuss a different room arrangement when its usual route does not fit current restrictions.

Ordered mobility practice

Caregivers need task-specific guidance as much as patients do. The plan should identify where help is required and whether the helper can provide it. Concerns about the incision, medication or other clinical changes belong with the appropriate treating professional. When new instructions arrive after a surgical follow-up, the rehabilitation plan should be reviewed against them rather than progressing by a calendar alone.

Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
