In-Home Physical Therapy in Cypress Park
Cypress Park is split in two by its own topography. The flats along San Fernando Road, Division Street, and the river side are level and industrial-edged; the streets climbing away from them ā Cypress Avenue, Isabel Street, Pepper Avenue, and the avenues above ā are steep, narrow, and in many places have no sidewalk at all. Which half you live on changes the therapy plan completely, so that is the first thing our licensed physical therapists establish.
Our therapists serve all Cypress Park neighborhoods, from the hillside streets down to the river flats, Cypress Avenue, Isabel Street, and Pepper Avenue, San Fernando Road, Division Street, and North Figueroa Street, and the avenue blocks in between.
When the Street Has No Sidewalk
On a lot of the hillside blocks, walking practice means walking in the roadway ā on a grade, on a narrow street, with parked cars blocking sightlines and drivers who do not expect a pedestrian moving slowly. Telling a patient in that situation to walk outside daily is not a plan.
What we do is decide, explicitly, where the outdoor work happens. Sometimes that is the flat ground below rather than the block outside the door, reached by car for the first few weeks. Sometimes it is the driveway and a measured indoor circuit until gait speed is high enough to make the street reasonable. Either way the location is named, the reasoning is explained, and the decision gets revisited as the measurements change ā rather than left to the patient to figure out alone.
What the Evaluation Covers
Your therapist measures strength, joint range of motion, standing balance, and gait speed, then measures the setting: the entry steps, any exterior staircase from street to door, the grade outside, whether a sidewalk exists, doorway widths against your device, furniture and bed heights, and the bathroom. Medications, blood pressure response to standing, and fall history are reviewed.
Conditions We Treat at Home
- Hillside and no-sidewalk gait training
- Recovery after knee, hip, and shoulder replacement or repair
- Balance deficits and fall prevention
- Gait retraining after stroke or with Parkinson’s disease
- Weakness and deconditioning after a hospital stay
- Back and joint pain limiting daily activity
- Caregiver training for the household, in Spanish or English
Visits That Fit a Working Household
Many families here have everyone in the house working, and the person who will be helping with transfers is not home at eleven in the morning. We schedule evening and weekend visits for that reason, and we make a point of doing the caregiver training when the caregiver is present rather than leaving printed instructions on a counter for someone who never saw the technique performed.
Coordination With Your Physicians
We coordinate with Barlow Respiratory Hospital, Los Angeles General Medical Center, Keck Hospital of USC, Adventist Health White Memorial, Glendale Memorial Hospital and Health Center, and PIH Health Good Samaritan Hospital. Your physician receives objective measurements and progress notes after visits.

Starting Physical Therapy Services
Home physical therapy requires a physician order documenting medical necessity. Medicare may cover in-home physical therapy if you qualify, which generally requires a physician order, a skilled therapy need, and homebound status as Medicare defines it. What you pay depends on your specific plan, and coverage is not guaranteed in advance. We accept Original Medicare Part A & B, Blue Shield of California PPO, and Anthem Blue Cross PPO.
Our intake team verifies your coverage and explains your benefits in Spanish or English before services begin, and we can contact your physician’s office directly to request the order.

All neighborhoods from the hillside streets down to the river flats ā Cypress Avenue, Isabel Street, and Pepper Avenue, the San Fernando Road, Division Street, and North Figueroa corridors, and the avenue blocks in between.
Somewhere we choose deliberately. That may be flat ground below rather than your own block for the first weeks, or the driveway and a measured indoor circuit until your gait speed makes the street reasonable. The location is named and revisited as your measurements change.
Yes, you can make that request. Share the caregiver's availability with intake and ask which appointment can be confirmed. Explain that they need to learn the prescribed assistance, so the therapist can plan the relevant teaching when they attend.
Yes ā along with Keck Hospital of USC, Adventist Health White Memorial, Glendale Memorial, and PIH Health Good Samaritan.
Walking practice can begin on an assessed indoor route when that is appropriate for your condition. The therapist can specify the distance, turns, rest points, and assistance needed. Progressing outdoors depends on both your ability and the route's conditions; it does not have to be the first step in rehabilitation.
