Medications, blood pressure response to standing, and fall history are reviewed.

We coordinate with Emanate Health Inter-Community Hospital, San Dimas Community Hospital, Emanate Health Foothill Presbyterian Hospital, Kindred Hospital San Gabriel Valley, and Emanate Health Queen of the Valley Hospital. For patients coming home after a hospital stay we work directly with discharge planners. Your physician receives objective measurements and progress notes after visits.
