A recovery plan should account for the person providing help. Caregiver support through social work examines practical strain, available assistance and resource questions that affect whether the home arrangement is workable.

Caregiver-needs discussion

Caregiver support examines whether the assistance expected at home is practical. The helper may have physical limitations, limited availability or concerns about coping with the responsibilities. These are relevant parts of the care arrangement, not assumptions to leave outside the clinical discussion.

Support arrangement review

Social work can assess support and resource needs with the patient's consent. Therapists separately teach assessed physical tasks, and nursing addresses clinical responsibilities. Keeping those roles clear prevents a support discussion from being mistaken for transfer training or authorization to perform a procedure. Family members may also need a shared understanding of who provides which help.

Resource options

Respite and other outside services have separate acceptance and availability requirements. Visit-based home health does not supply continuous replacement care. If the caregiver becomes unavailable or cannot provide the required help, the team needs to review the actual arrangement. The patient should not be assumed able to manage alone simply because a support option has been discussed.

Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
