Arranging Care After a Hospital Stay

The simplest route is to ask the hospital discharge planner to refer you before you leave — we coordinate directly with Providence Holy Cross Medical Center, Northridge Hospital Medical Center, Kaiser Permanente Panorama City, Valley Presbyterian Hospital, and Mission Community Hospital. If you are already home, your physician can send an order and we can contact their office to request it. Most patients begin within one to two days of a completed referral.

Who Provides the Care

Treatment is provided by licensed physical therapists, with licensed physical therapist assistants working under a therapist-directed plan of care where appropriate. Your evaluation and all plan-of-care decisions are made by the licensed therapist, who also performs the reassessments.

Between Visits

You are given a direct number and a specific list of what warrants using it — a fall or near-fall, new or sharply increased pain, new swelling or calf tenderness, dizziness on standing, or a sudden change in what you can do. A clinician responds, and we contact your physician when it needs one.
