This Privacy Policy explains how HarvardCare Home Health handles information collected through this website, including requests about in-home physical therapy and other home health services. It covers website visitors, people making inquiries, and professionals submitting referrals.
Patient health information may be protected by HIPAA and California medical privacy law. Our Notice of Privacy Practices explains those rights and our healthcare privacy duties. This website policy does not replace that notice, authorize a disclosure of health information, or reduce your legal rights.
1. Information collected through the website
- Inquiries: the name, phone number, email address, city or ZIP code, requested service, coverage information, and message you provide. Different forms request different details.
- Professional referrals and intake: the referring professional’s name, organization, role, and contact details; patient identity and contact information; date of birth; address; insurance information; requested services; provider and referral details; and documents or notes submitted through the designated intake process.
- Website activity: IP address, browser and device information, approximate location inferred from an IP address, landing page, referring page, campaign parameters, advertising click identifiers, timestamps, and interactions such as clicking a telephone link or submitting a form. A telephone-link click is an interaction record; it does not itself contain a recording of a call.
- Preferences and technical information: cookies and browser storage used for website functions, accessibility preferences, and the tracking described in our Cookie Policy.
Information comes from you, an authorized person or referring professional, and the browser or device used to visit the site. Please provide only the details requested. Do not put medical documents, insurance member numbers, or sensitive clinical information in a general contact message, URL, or ordinary email. Contact our office for the appropriate intake method.
2. Why information is used
We use inquiry and referral information to respond to requests, contact the appropriate person, review service availability and eligibility, coordinate referrals, and support care-related administration. Health information is used or disclosed only as permitted by the applicable healthcare privacy rules.
Technical and activity information supports site operation, security, troubleshooting, inquiry handling, and measurement of how people find and use the website. The site’s attribution tools can associate a campaign source with an inquiry or telephone-link click. Microsoft Advertising tracking supports advertising measurement and related advertising functions.
3. Who may receive information
- Agency personnel and service providers: people who handle inquiries, referrals, site hosting, technical support, communications, and other functions needed to operate the service.
- Healthcare providers and payers: when information is needed for permitted care coordination, eligibility checks, payment, or healthcare operations.
- Website and advertising providers: Microsoft Advertising may receive device identifiers, IP address, page information, and interaction data through its website tag. An IP-location service may receive an IP address to estimate a visitor’s city or region. Providers of fonts, map tiles, and other requested web resources also receive the technical information needed to deliver those resources.
- Other recipients: when required or permitted by law, for legally authorized protection of rights and security, or with an authorization or consent where the law requires one.
Advertising providers may connect activity with information from other websites or services under their own policies. An advertising disclosure can constitute a āsaleā or āsharingā under California law even when no money is exchanged. We do not treat acceptance of a website policy or cookie choice as permission to disclose protected health information for advertising.
When a vendor performs a function that makes it a HIPAA business associate, the applicable HIPAA permission and business associate agreement requirements apply.
4. Cookies and browser privacy signals
The Cookie Policy identifies the site’s enabled tracking and preference technologies and explains browser and provider controls. The current website does not offer an on-site cookie preference panel and does not automatically change tracking in response to Do Not Track or Global Privacy Control signals. A browser setting or provider opt-out may have a limited scope and may not remove information already collected.
5. Requests and California privacy rights

We may need to verify your identity and, if you act for another person, your authority. Please do not send identity documents until we have arranged an appropriate method.
Where the California Consumer Privacy Act, as amended by the California Privacy Rights Act, applies to a business and the information involved, it provides rights to know and access personal information, request correction or deletion, opt out of sale or sharing, limit certain uses of sensitive personal information, and exercise rights without unlawful discrimination. Authorized agents may submit qualifying requests. Exceptions can apply to healthcare information and legally required records; HIPAA-covered status does not automatically exempt every item collected by a website.

6. Retention and security
Retention depends on the information’s purpose, whether an inquiry becomes a patient record, applicable clinical and administrative record requirements, and any legal obligation to preserve it. Browser-cookie expiry is not the same as deletion of a server-side record. Ask our office about retention or deletion for a particular record.
Technical safeguards can reduce risk, but no website, email service, or transmission method guarantees complete security. Please use the communication method arranged with our office for sensitive information.
7. Children and links to other sites
This website is intended for adults seeking information or coordinating care. A parent, legal guardian, or other authorized representative should contact us on behalf of a child. If you believe a child has submitted information without appropriate authorization, let us know so we can review it. This statement does not restrict any rights minors have under applicable law.
External websites have their own privacy practices. Review their notices before providing information. This policy applies to HarvardCare’s website, not every service reached through a link.
8. Changes and contact
When we revise this policy, we post the updated version and change its effective date. For material changes, we provide additional notice or obtain consent when the law requires it. A policy update does not by itself authorize a new use of protected health information.
