Skilled Nursing Care at Home: an individual home care plan

Recovery at home can involve clinical questions alongside movement and daily activities. Skilled nursing addresses ordered assessment, teaching and care needs while sharing relevant information with the therapist and treating clinician.

Clinical assessment

Skilled nursing has a defined place in a coordinated recovery plan. The nurse assesses and addresses ordered clinical needs that may affect how the patient manages at home. Teaching, medication-plan clarification and communication with the treating clinician can sit alongside movement and daily-activity work from PT and OT.

Medication-plan review

The nursing assessment determines the relevant tasks rather than assuming every referral needs the same procedure. A clinical concern may require clarification of an order, a change directed by the prescriber or another professional's review. Nursing and therapy should share restrictions and significant changes so their instructions do not conflict.

Patient and caregiver teaching

Visit-based care also needs clear arrangements between appointments. The patient and caregiver should understand their assigned responsibilities and whom to contact. Scheduled visits do not provide continuous attendance or emergency-response coverage. Specialized procedures require a separate review of orders, suitability, supplies and qualified staffing before they are confirmed.

Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
