These Terms of Use describe the conditions for using HarvardCare Home Health’s website. The site provides information about in-home physical therapy and other home health services in Los Angeles County and offers ways to contact the agency or submit a professional referral.
1. Website information and care requests
Website content is general information, not a diagnosis, an individual treatment plan, or a substitute for advice from your clinician. Follow your healthcare provider’s instructions and ask that provider whether an activity or service is appropriate for you. See our Disclaimer.
For a medical emergency, call 911. Website forms and email are not emergency services.
Sending a message or referral does not by itself establish a provider-patient relationship, book a confirmed visit, or guarantee admission. The agency must review the request, service availability, clinical needs, required orders, and eligibility. Any care relationship and services are governed by the applicable admission, consent, and care documents.
2. Eligibility, coverage, and availability
Descriptions of Medicare, insurance, service areas, or visit timing are general information. Coverage, authorization requirements, patient costs, staffing, and start-of-care timing depend on the individual circumstances and must be confirmed. A form confirmation means the request was received; it is not an approval of benefits or services.
3. Appropriate use
You may use the website to learn about services, read resources, contact the office, and make authorized referrals. Provide accurate information and submit information about another person only when you have the authority and legal basis to do so.
Do not misuse forms, impersonate someone, submit fraudulent information, introduce malicious code, attempt unauthorized access, collect other people’s private information, or disrupt the site. These rules do not prohibit lawful accessibility tools, ordinary search indexing, or uses protected by law.
4. Information you submit
Submit only information needed for your request. General contact forms and ordinary email should not be used to send medical records, insurance member numbers, or other sensitive documents. Contact the office to arrange the appropriate intake method. Professional referral tools are intended for authorized referral sources.
You retain any rights you have in your submissions. Providing a message or referral permits us to process it for the relevant inquiry or care-coordination purpose, subject to applicable privacy law. It does not give us a general license to publish, market, or reuse your health information. A form acknowledgment or website visit is not a HIPAA authorization.
5. Content and intellectual property
The agency’s branding and website materials, and third-party materials used on the site, may be protected by copyright, trademark, or other rights. You may read and print information for personal or authorized care-coordination use, subject to applicable law. Obtain permission for uses that require a license, and do not imply endorsement or affiliation that does not exist. Statutory exceptions and other rights permitted by law remain available.
6. External resources
Links to outside services are provided for convenience. Those services control their own content, availability, terms, and privacy practices. An external link does not create an endorsement or a clinical recommendation.
7. Website availability and legal rights
We may correct or update website content, change features, or restrict misuse of the site. Information may contain errors or become outdated, and uninterrupted access is not guaranteed. Contact the office to confirm details that affect a care decision.
Nothing in these Terms excludes liability that cannot lawfully be excluded, excuses fraud or unlawful conduct, waives patient privacy or consumer rights, or replaces duties owed in providing healthcare. These Terms do not impose a contractual damages cap on your care, mandatory arbitration, or a waiver of the right to contact a regulator.
8. Privacy and changes to these Terms
Our Privacy Policy explains website data practices, and our HIPAA Notice explains healthcare privacy rights. The Cookie Policy describes browser storage and tracking. Using the site does not waive protections provided by those laws.
We post revisions here with an updated effective date. Material changes are subject to any notice or consent requirements that apply. California law and applicable federal law govern these Terms, without taking away nonwaivable rights available under another applicable law.

You may contact us to discuss a website issue. Doing so is not a prerequisite to exercising a legal right, filing a complaint, or seeking an available remedy.
