Physical Therapy That Comes to You in Pacific Palisades

Nestled between the Santa Monica Mountains and the Pacific, Pacific Palisades is built across canyon and bluff, and almost every address involves grade. The Riviera and Huntington Palisades sit on rolling terrain with houses set above or below the street. Castellammare clings to the slope above PCH on switchback roads. The Palisades Highlands and Marquez Knolls climb into the hills on long curving approaches, and Santa Monica Canyon runs below with its own stepped streets. Only the Village and the Alphabet Streets offer anything like level walking. Our licensed physical therapists build the plan around the grade you actually have.

Our therapists serve Pacific Palisades Village and the Alphabet Streets, The Riviera, Castellammare, and Huntington Palisades, the Sunset Boulevard corridor, Via de la Paz, and Temescal Canyon areas, Santa Monica Canyon and the blufftop neighborhoods, and Marquez Knolls and Palisades Highlands.

Why Home Beats the Clinic Here

Outpatient therapy is good care, and for many patients it is the right setting. But it has one structural weakness that matters enormously in the Palisades: it trains you on level flooring with handrails at hand, and then sends you home to a canyon driveway. A patient can meet every clinic goal and still be unable to get from their car to their front door.

In-home therapy removes the drive on canyon roads through coastal traffic, and more importantly it trains on the actual approach — your grade, your steps, your surface, in the lighting you actually have at six in the evening.

What the Evaluation Covers

Your therapist measures strength, joint range of motion, standing balance, and gait speed, then applies them to the property: driveway grade in both directions, entry steps, interior stairs and level changes, the bathroom transfer, and the distance to where a vehicle stops. Medications, blood pressure response to standing, and fall history are reviewed.

Conditions We Treat at Home

  • Recovery after knee, hip, and shoulder replacement or repair
  • Controlled descent and loaded ascent on canyon grades
  • Balance deficits and fall prevention on sloped properties
  • Gait changes after stroke or with Parkinson’s disease
  • Deconditioning after hospitalization or illness
  • Back, neck, and joint pain limiting an active routine
  • Return to hiking Temescal and the local trails after injury

If You Are Not Sure You Qualify

Medicare home health generally requires a physician order, a skilled therapy need, and homebound status as Medicare defines it. Homebound does not mean bedbound — it generally means leaving home takes a considerable and taxing effort and that you need help or a device. People do leave for medical appointments and occasional outings and still meet it, and a canyon driveway is itself relevant to that assessment.

Whether you qualify is determined with your physician. We will evaluate and report honestly, and if we think the criteria are not met we will say so and discuss outpatient alternatives with your ordering clinician.

Weekend Availability

Weekend visits are available, which matters when a family member who works during the week is the person who needs to learn the guarding and transfers.

Coordination With Your Physicians

We coordinate with UCLA Medical Center Santa Monica, Providence Saint John’s Health Center, UCLA Health Pacific Palisades, and Cedars-Sinai Medical Center. Your physician receives objective measurements and progress notes after visits.

Contact intake with your preferred timing and ask whether a suitable weekend appointment can be confirmed. Scheduling depends on your clinical requirements and clinician availability. Make separate arrangements for any help you need between visits.
