Caregiver Training at Home: an individual home care plan

A caregiver needs more than a general instruction to help. Hands-on training addresses an assessed transfer or daily task, the patient's restrictions and the helper's own ability to provide the required support.

Assessed assistance methods

Hands-on caregiver training begins with a defined activity. For a transfer, that includes the surfaces, equipment, patient's restrictions and required assistance. For a daily-care task, it includes the patient's participation and the specific help needed. The therapist also considers the caregiver's strength, reach and ability to follow the instructions.

Transfer setup

Demonstration alone is not the whole process. Where appropriate, the helper can practice the agreed method under professional guidance and ask about uncertain steps. The therapist can identify where a direction has been misunderstood or the proposed arrangement is not practical. A belt, walker or other device does not replace this individual review.

Guided caregiver practice

Training should be revisited when circumstances change. A new medical restriction, different helper or altered transfer ability may mean the previous method no longer fits. This service addresses task performance; broader caregiver education can explain routines and communication, while social work may assess resource needs. None of those services should assume a relative can provide assistance beyond their capacity.

Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
