In-Home Physical Therapy in West Athens
West Athens is an unincorporated community of level residential blocks between the Western and Normandie corridors, across the Denker, Budlong, and Halldale neighborhoods, and along the numbered streets off Century Boulevard and Imperial Highway. The houses are small and single-story. A substantial share of the patients we see here are living with COPD or another chronic lung condition, and home therapy for those patients involves something that is almost never discussed: the equipment itself. Our licensed physical therapists start there.
Our therapists serve all West Athens residential blocks, the Western Avenue and Normandie Avenue corridors, the Denker, Budlong, and Halldale Avenue neighborhoods, the homes off Century Boulevard and Imperial Highway, and the numbered residential streets in between.
Fifty Feet of Oxygen Tubing in a Small House
A concentrator with a long tube is what allows a patient to move around their own home, and it is also a continuous loop of line lying across every floor they cross. It catches on chair legs, loops under doors, tangles round a walker, and lies exactly where a foot lands on the way to the bathroom at night. It is one of the most common trip hazards we see and it is rarely named, because it is treated as part of the furniture.
The fixes are practical: routing the line along a wall rather than across a room, shortening it to what the used route actually needs, hooks or clips at the corners it wraps around, and a deliberate technique for managing it while turning with a walker. We also teach the breathing work that makes movement possible in the first place ā pursed-lip breathing, pacing an effort so it is not one long push, and coordinating breath with the hardest part of a movement such as standing from a chair ā and we set exertion targets you can check yourself rather than telling you to take it easy.
What the Evaluation Covers
Your therapist measures strength, joint range of motion, standing balance, gait speed, and exertion tolerance with recovery time, then measures the home: the oxygen tubing route and length, the path from bed to bathroom, doorway widths against your device, furniture and bed heights, the entry steps, and the indoor distance available for a circuit. Medications, breathing pattern, blood pressure response to standing, and fall history are reviewed.
Conditions We Treat at Home
- Conditioning and breathing technique for COPD and chronic lung disease
- Fall prevention including equipment and tubing hazards
- Recovery after knee, hip, and shoulder replacement or repair
- Balance deficits
- Gait retraining after stroke or with Parkinson’s disease
- Weakness and deconditioning after a hospital stay
- Caregiver training for the household, in Spanish or English
Short Efforts, Repeated
For patients who become breathless quickly, one long session is the wrong shape. We build the program as several short efforts spread through the day, with recovery time measured rather than guessed, because how quickly you recover is a better early signal than how hard something felt. Distance work uses a measured indoor circuit first, moving outdoors to the block once the numbers support it ā and we tell you plainly which symptoms mean stop and call your physician rather than push on, because that distinction is most of what keeps someone out of hospital.
Coordination With Your Physicians
We coordinate with Memorial Hospital of Gardena, Kindred Hospital South Bay, MLK Community Hospital, Centinela Hospital Medical Center, Crenshaw Community Health Center, and St. Francis Medical Center. Your physician receives objective measurements and progress notes after visits, and we call directly when a breathing pattern needs their decision.

Starting Physical Therapy Services
Home physical therapy requires a physician order documenting medical necessity. Medicare may cover in-home physical therapy if you qualify, which generally requires a physician order, a skilled therapy need, and homebound status as Medicare defines it. What you pay depends on your specific plan, and coverage is not guaranteed in advance. We accept Original Medicare Part A & B, Blue Shield of California PPO, and Anthem Blue Cross PPO.
Our intake team verifies your coverage and explains your benefits in Spanish or English before services begin, and we can contact your physician’s office directly to request the order.

It is one of the most common ones we see and almost nobody names it. Fifty feet of line loops under doors, catches chair legs, tangles round a walker, and lies where your foot lands on the way to the bathroom at night.
Route it along a wall rather than across rooms, shorten it to the route you actually use, add hooks at the corners it wraps around, and learn a deliberate technique for managing it while turning with a walker.
As several short efforts through the day rather than one long session, with recovery time measured ā how fast you recover is a better early signal than how hard it felt. We also teach pursed-lip breathing and coordinating breath with the hardest part of a movement.
Tell the therapist about your prescribed oxygen and the instructions from your medical team. The exercise plan can account for tubing, equipment placement, rest, and symptoms. Do not change the prescribed flow yourself to complete an activity; questions about the prescription belong with the clinician managing it.
