In-Home Physical Therapy in Signal Hill
Signal Hill is exactly what its name says: a hill, rising out of Long Beach on every side, with Crescent Heights and the older streets near Hill Street, the Cherry and Atlantic Avenue corridors, Hilltop Park and the Skyline and Panorama Drives, the Willow, Spring, and Orange blocks below, and Discovery Well Park to the east. The geography here is unlike anywhere else we serve, and it creates a problem our licensed physical therapists have to solve before writing any walking program.
Our therapists serve Crescent Heights and the older streets near Hill Street, the Cherry Avenue and Atlantic Avenue corridors, Hilltop Park, Skyline Drive, and Panorama Drive, Willow Street, Spring Street, and Orange Avenue, and the Discovery Well Park area.
From the Top, Every Direction Is Downhill
In most neighborhoods a sloped street has a flat one nearby. On a dome there is no flat direction at all. Leave a house near the crest and you descend whichever way you go, which means every single return is a climb ā and the climb arrives at the end, when you are most tired and furthest from a chair. Patients discover this the hard way on their first attempt and often do not try again.
The answer is to stop treating the block outside the door as the default. For patients living high on the hill, early distance work happens on level ground reached by car, or on the level loop at the park, while the driveway and the first stretch of the street are trained separately as their own short, supervised task. As gait speed and endurance improve we extend the outdoor route deliberately, always with the return leg planned as part of the effort rather than as an afterthought. Patients in the flatter blocks below get a conventional graded program, and we are clear about which of the two situations you are in.
What the Evaluation Covers
Your therapist measures strength, joint range of motion, standing balance, and gait speed, then measures the setting: the grade outside your door in each direction, the driveway pitch, the distance and climb from the house to where a car stops, the entry steps, doorway widths against your device, furniture and bed heights, and the bathroom. Medications, blood pressure response to standing, and fall history are reviewed.
Conditions We Treat at Home
- Graded-slope and sustained-incline training
- Recovery after knee, hip, and shoulder replacement or repair
- Balance deficits and fall prevention
- Gait retraining after stroke or with Parkinson’s disease
- Weakness and deconditioning after a hospital stay
- Back and joint pain limiting daily activity
- Caregiver training for the household
Descending Is Its Own Skill
Walking downhill is not the easy half. It loads the quadriceps eccentrically ā the muscle lengthens under tension to control the descent ā which is precisely what is weakest after a knee operation or a long hospital stay, and it is why patients report their legs shaking on the way down rather than on the way up. It also shifts the body forward over the feet, which is a poor position for anyone with balance problems. We train descent explicitly: shorter steps, a slight zigzag across the slope where the pavement allows, a cane placed downhill, and controlled speed rather than letting gravity set the pace.
Coordination With Your Physicians
We coordinate with College Medical Center, MemorialCare Long Beach Medical Center, Dignity Health St. Mary Medical Center, the Tibor Rubin VA Medical Center, Lakewood Regional Medical Center, and the urgent care providers serving the area. Your physician receives objective measurements and progress notes after visits.

Starting Physical Therapy Services
Home physical therapy requires a physician order documenting medical necessity. Medicare may cover in-home physical therapy if you qualify, which generally requires a physician order, a skilled therapy need, and homebound status as Medicare defines it. Coverage is not guaranteed in advance. We accept Original Medicare Part A & B, Blue Shield of California PPO, and Anthem Blue Cross PPO.
Our intake team verifies your coverage and explains your benefits before services begin. Most patients start within one to two days of a completed referral.

Because from near the crest every direction is downhill, so every return is a climb ā and it arrives at the end when you are most tired and furthest from a chair. Patients discover that on the first attempt and often do not try again.
Early on, level ground reached by car or the loop at the park, while your driveway and the first stretch of street are trained separately as a short supervised task. We extend the route as your measurements support it.
Because descending loads the quadriceps as it lengthens under tension, and that is exactly what is weakest after a knee operation or a hospital stay. Downhill is not the easy half, and we train it explicitly.
The two directions require different control and may occur at different levels of fatigue. Surface and support also matter. The therapist can assess each direction and explain appropriate help rather than assume that completing the climb establishes readiness for the descent.
