What Homebound Actually Means

This is the question we are asked most often here, usually by patients who are active, independent, and assume they cannot qualify. Medicare’s homebound criterion does not require being bedbound or housebound. It generally means that leaving home takes a considerable and taxing effort, and that you need help or an assistive device to do it. People do leave home for medical appointments, religious services, and occasional outings and still meet the criterion.

Whether you qualify is a clinical determination made with your physician, not something we can promise in advance. What we can do is assess you honestly and tell your physician what we find.

What the Evaluation Covers

Your therapist measures strength, joint range of motion, standing balance, and gait speed, then applies them to your property: the entry steps, the driveway grade in the canyon neighborhoods, interior stairs, the bathroom transfer, and the distance to where a vehicle can stop. Medications, blood pressure response to standing, and fall history are reviewed.

Continuity Between Clinicians

Where more than one clinician is involved in your care, the plan of care is held by one licensed physical therapist who performs the evaluation and sets the program. Any therapist assistant works under that plan, with the same measurements, the same progressions, and the same documentation. You are told who is coming and when, and the therapist who evaluated you performs the reassessments.

Coordination With Your Physicians

We work with UCLA Medical Center, Providence Saint John’s Health Center, UCLA Health Brentwood, and the VA West Los Angeles Healthcare Center, and we collaborate readily with concierge and boutique practices — many Brentwood patients use them, and we communicate in whatever form the practice prefers. Your physician receives objective measurements and progress notes after visits.

Home physical therapy requires a physician order documenting medical necessity. Medicare may cover in-home physical therapy if you qualify, which generally requires a physician order, a skilled therapy need, and homebound status as Medicare defines it. Coverage is not guaranteed in advance. We accept Original Medicare Part A & B, Blue Shield of California PPO, and Anthem Blue Cross PPO.
