Balance Only Improves If It Is Challenged

Standing on one leg while holding the kitchen counter with both hands is not balance training once it has become easy. Then the difficulty gets raised deliberately — feet together, then one foot in front of the other, then one leg; eyes open, then with the head turning; on the floor, then on a cushion. Each step is chosen so that you can catch yourself, and the progression is measured rather than guessed.

We are equally clear about the opposite error. Balance work done alone, in the middle of a room, with nothing in reach, is not brave — it is how someone ends up on the floor. We set the conditions under which each exercise may be performed between visits, including which ones need another person present, and we write that down.

What the Evaluation Covers

Your therapist measures strength, joint range of motion, standing balance in several conditions, gait speed, and how you respond when your balance is disturbed, then measures the home: where in the house a balance exercise can be performed safely, the staircase and its rail, the entry steps, the driveway grade, doorway widths, furniture and bed heights, and the bathroom. Medications, blood pressure response to standing, vision, and fall history are reviewed.

Hills, Heat, and a Program That Holds

Outside, the streets curve and climb, and for most patients here the return leg is the limiting factor rather than the distance. We set the route with direction and turnaround specified, and hold the steeper sections back until the measurements support them. Summer heat is the other constraint — it affects balance and blood pressure directly rather than being merely uncomfortable — so the outdoor portion moves to early morning for much of the year, with a measured indoor circuit and the balance progression covering the days when outside is not sensible. The distance to the nearest hospital is also real, which is much of the practical case for treating people at home rather than asking them to drive twice a week.
