Home Modification Recommendations: an individual home care plan

Functional home recommendations begin with the movement or activity that is difficult. OT can assess the need for changes to access, support or task setup, while installation, property permissions and construction remain separate responsibilities.

Functional access assessment

Home-modification recommendations should begin with a functional problem. The therapist examines the task, available space, equipment and assistance before discussing a change. A proposed ramp, bar or room arrangement needs to relate to that assessment rather than a general expectation about disability or age.

Task-based recommendations

The responsibilities remain distinct. OT can describe clinical and functional needs; property owners and qualified professionals address permissions, structural design and installation. A recommendation does not certify that a change can be built or that it will be paid for. Equipment and modification funding need their own review before commitments are made.

Equipment and layout discussion

Practical constraints should be discussed openly. The household may share rooms, rent the property or be unable to implement a particular suggestion. The team can consider alternatives or relevant resource questions without assuming the recommended work is already complete. After a change, the patient's functional use and need for instruction may also require reassessment.

Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
