Swallowing Therapy at Home: an individual home care plan

Swallowing therapy follows an individual evaluation and clinical plan. The speech-language pathologist considers the diagnosed difficulty, current eating and drinking instructions and the practical support available during meals.

Assessment-led swallowing care

Swallowing therapy follows appropriate assessment and an individual clinical plan. The clinician considers the known difficulty, relevant medical information and current eating and drinking directions before selecting treatment. A general exercise or online strategy should not be treated as suitable for every swallowing problem.

Current meal instruction review

The meal routine also matters. Caregivers need to understand the instructions they are expected to follow and the assistance assigned. Food or fluid changes require appropriate clinical direction. Nursing and other ordered services should receive relevant information so the household is not given conflicting recommendations.

Caregiver teaching

A home visit cannot resolve every question about swallowing. Further medical or instrumental evaluation may be needed, and urgent concerns require the appropriate response rather than waiting for a scheduled session. Treatment does not guarantee return to a previous diet or elimination of risk. The plan is reviewed against the person's condition and assessment findings.

Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
