Post-stroke care-plan review

Post-stroke home care can involve several disciplines, but not every patient requires all of them. The referral and assessment identify current movement, daily-activity, communication and clinical needs. Services should be selected from that individual picture rather than from a standard stroke package.

Multidisciplinary assessment

Instructions need to fit together. Communication strategies may help the patient understand PT or OT directions, while mobility restrictions can affect personal-care assistance. Swallowing concerns require their own assessment and current instructions. With consent, caregivers can explain which tasks are difficult and whether the expected assistance is practical in the home.

Caregiver role planning

The treating prescriber remains responsible for medication decisions, with nursing clarifying and communicating relevant concerns when ordered. Equipment and support gaps need review rather than an assumption that discharge paperwork resolves them. Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
