Personal Care Assistance at Home: an individual home care plan

Personal care assistance should match the individual routine: what the patient can do, what requires help and which precautions apply. Within home health, assigned aide tasks support the assessed care plan rather than replace skilled rehabilitation.

Personal routine assessment

Personal care assistance begins with the activity and the person's current abilities. The assigned plan identifies which portions require help and where participation remains appropriate. A diagnosis alone does not describe the assistance needed for washing, clothing or another routine, and the patient's preferences should be part of the discussion.

Assigned task assistance

Skilled assessment and assistance have different roles. OT can evaluate and teach a method, while an aide may provide the help specified in the plan. Nursing or therapy precautions need to be communicated clearly. The task should not be changed informally because a helper prefers a different approach.

Dignity and preference

The service scope must also be understood. Home health personal assistance is tied to applicable eligibility and the assigned skilled plan; it is not an unlimited domestic service or a guarantee of long-term custodial care. If the person needs more help or the usual caregiver becomes unavailable, the arrangements require review rather than assuming visits cover the gap.

Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
