Patient Education at Home: an individual home care plan

Understanding an instruction is different from receiving a handout. Patient education through nursing addresses the ordered clinical plan, checks what the patient or caregiver understands and identifies questions that need clarification.

Clinical instruction review

Nursing education is a skilled activity when it involves clinical judgment related to an assessed need. The nurse reviews what the patient has been told, which responsibilities apply and where understanding is uncertain. A handout can support the discussion, but receiving written instructions does not establish that they can be used correctly.

Understanding checks

The approach should account for practical barriers. A patient may understand a direction yet have difficulty handling equipment, remembering a sequence or arranging help. The nurse can identify those issues and coordinate relevant assessment rather than simply repeat the instruction. Communication needs should be discussed so suitable arrangements can be confirmed.

Caregiver role discussion

Teaching follows the current clinical plan. It does not give a relative independent authority to perform a procedure or change treatment. With consent, the responsible caregiver can learn appropriate tasks and reporting directions. Questions outside nursing's assigned role should go to the appropriate treating clinician, with information shared across ordered recovery services when relevant.

Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
