Injections at Home (Nurse): an individual home care plan

An injection request needs a medication-specific review. Before care is arranged, the agency must confirm the order, clinical suitability, medication responsibilities and qualified staffing rather than treat all injections as interchangeable.

Medication-order review

An injection referral must identify the medication and complete prescribing directions. Different drugs, routes and clinical circumstances require different review. The agency needs to confirm suitability, medication and supply arrangements, and qualified staffing before accepting a home visit.

Staffing and suitability check

The patient and family should understand who supplies the medication and who makes treatment decisions. A delayed delivery or unclear dose needs communication with the responsible providers. It does not authorize the household to change timing, route or medication on its own. A therapy visit cannot substitute for appropriately arranged medication administration.

Supply responsibility confirmation

Any caregiver responsibility must be explicitly defined and taught by the clinical team. Watching a procedure is not proof of training or authorization. Medication benefits and professional-visit benefits also require individual review. Scheduled nursing is distinct from emergency treatment, so urgent concerns must follow the emergency instructions provided for the patient's situation.

Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
