Infected Wound Care at Home: an individual home care plan

An infection concern needs appropriate medical assessment and a clear treatment plan. Home nursing, when accepted and ordered, supports follow-up and communication while rehabilitation follows the current clinical restrictions.

Medical-plan clarification

An infection-related wound concern requires appropriate medical assessment. A scheduled home health visit is not a substitute for urgent or emergency evaluation when the situation warrants it. Once a treating plan is established and the referral accepted, nursing may address ordered follow-up, teaching and communication of relevant changes.

Ordered follow-up assessment

Prescribing decisions remain with the treating clinician. The household should not change an antibiotic, add an antiseptic or assume a different product will address a new concern. IV treatment, if requested, requires separate orders, medication and supply arrangements, clinical suitability and qualified staffing confirmation. It is not automatically included with infection-related follow-up.

Prescriber communication

Rehabilitation must follow the current medical restrictions. The therapist should receive relevant updates before affected activity is considered. Caregiver observations can inform the clinical team but do not independently establish that an infection has resolved. The plan may require additional evaluation or a different setting, and no promise is made that treatment will be completed successfully at home.

Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
