In-Home Speech Therapy: an individual home care plan

A useful speech-language assessment begins with a real communication or swallowing concern. The first home evaluation gathers the clinical context, observes relevant tasks and identifies whether an individualized skilled plan is appropriate.

Referral preparation

The first home speech-language evaluation begins with the reason for referral. Medical history, existing assessments and current instructions give the clinician necessary context. The patient and, with consent, a caregiver can describe specific communication or swallowing situations that have become difficult.

Functional speech evaluation

Assessment tasks are selected for those concerns rather than taken from one universal test sequence. Everyday conversations, existing communication aids and relevant routine observations can help show the functional effect. A home examination also has limits: some swallowing or medical questions may require additional evaluation through another clinical setting.

Individual goal discussion

The findings guide discussion of goals and next steps. An evaluation does not promise a particular treatment course or automatically establish eligibility. Before any home practice is assigned, the patient and helper need understandable instructions. Eligibility and insurance review

Original Medicare Part A & B may cover eligible home health care when the applicable homebound, skilled-care, clinician-order and certification requirements are met and services follow an individual plan through a Medicare-certified agency. Coverage and any patient responsibility require verification. Blue Shield of California PPO and Anthem Blue Cross PPO benefits are also reviewed individually. Equipment, medications and separately billed services require their own benefit review.
