CARE THAT FITS YOUR NEEDS

Skilled Support for Life at Home

When walking across a room, getting out of a chair, or managing care after a hospital stay becomes difficult, the right support can make daily routines feel more manageable. Home health brings licensed clinicians to you for care ordered by your doctor or another qualified healthcare provider.

Your plan may include physical therapy for movement and balance, occupational therapy for everyday tasks, or skilled nursing for medical needs such as wound care. The services you receive depend on your assessment and care plan.

Visits can support recovery or help maintain function when skilled care is needed. Home health is different from a caregiver staying with you for companionship, housekeeping, or ongoing personal assistance. We help you understand which type of support fits your situation.

Your Care Plan May Include:

  • Physical therapy for walking, balance, strength, and transfers
  • Occupational therapy for safer everyday activities
  • Skilled nursing for symptoms, medications, and ordered treatments
  • Support during the transition from hospital to home
  • Practical instruction for patients, families, and caregivers
  • Assessment of fall risks and movement around your home
  • Updates and care coordination with your healthcare provider

UNDERSTANDING MEDICARE

Is Home Health Care Right for You?

Original Medicare pays for covered home health visits when you meet its requirements. Medical equipment has separate cost rules: typically 20% of the Medicare-approved amount after the Part B deductible. We review eligibility and discuss potential charges before care begins.

Provider’s Assessment: A qualified provider assesses you face-to-face, certifies your need, and orders your care.

Homebound Status: You normally cannot leave without considerable effort and need help or should not leave for medical reasons. Medical visits and short, infrequent outings may still be allowed.

Skilled Need: Your provider identifies a qualifying need for skilled nursing or therapy.

Scheduled Skilled Visits: Home health provides visits under a care plan. It does not cover round-the-clock care or personal assistance alone.

Let’s Review Your Coverage

We accept Original Medicare Part A & B, Blue Shield of California PPO, and Anthem Blue Cross PPO. Coverage, benefits, and eligibility are verified before admission.
